Awarded in August 2025, RISE strengthens prevention, care, and treatment services through integrated, community-led service delivery models aligned with Ministry of Health guidelines and U.S. Government priorities.
Project Goal: Accelerate progress toward HIV epidemic control in Eswatini by reducing new HIV infections, eliminating mother-to-child transmission (MTCT), and improving treatment outcomes among pregnant and breastfeeding women (PBFW), children, and adolescents.
The RISE Project is implemented by Young Heroes through Jhpiego with U.S. Government funding.
Target Groups
RISE prioritizes:
- Pregnant and Breastfeeding Women (PBFW)
- HIV-Exposed Infants (HEIs)
- Children and Adolescents Living with HIV (CALHIV)
- Sexual partners of PBFW
- People Living with HIV (PLHIV) requiring adherence and retention support
Geographic Coverage
The project is implemented across 34 Tinkhundla in all four regions of Eswatini:
Core Implementation Pillars
- HIV Prevention and Case Finding: Risk assessments; prevention counseling; HIV testing; follow-up and linkage to ART for positive cases.
- Prevention of Mother-to-Child Transmission (PMTCT): Enroll PBFW in structured community support groups; interventions for HIV-positive mothers and for HIV-negative mothers.
- Early Infant Diagnosis (EID): Long-term monitoring of HIV-exposed infants from birth to 24 months; support for prompt birth testing and interval testing; linkage to health facilities; active case tracking.
- PrEP Scale-Up and HIV Prevention for PBFW: Risk screening; PrEP literacy for eligible clients; initiation and retention support for oral PrEP; introduce and promote long-acting injectable PrEP; individual counselling, peer engagement, and digital follow-up.
- Care and treatment for children and adolescents living with HIV: support for ART adherence; monitoring viral load and tracking suppression; tracing treatment interruptions; peer support, including teen clubs and Sivivane Camp.
- Integrated Health Services: TB symptom screening and referral; cervical cancer screening assessment and linkage; community mobile clinics and medication pickup points; STI services, contraception, and condom distribution.
